In addition to completing the application posted within this site, other documents are required. All of the following items must be submitted or completed during the course of completing the foster/adoptive home study. Your worker will provide assistance as needed.

􀂄 SLED/Protective Service Authorizations

􀂄 Fingerprint Report for each member of the household who is age 18 and above.

􀂄 Discipline Agreement form

􀂄 Financial Information form

􀂄 Family History form

􀂄 Copy of last year’s W-2, tax return or the last three month’s pay stubs

􀂄 Fourteen hours of training

􀂄 Disaster Preparedness Plan

􀂄 Autobiography (one from each applicant)

􀂄 Medical Report (one on each spouse, if married, and any adult members of your household)

􀂄 Medical Report on any minor children, under age 18, living in your home other than foster children

􀂄 Child Factors Checklist

􀂄 Openness Checklist

􀂄 Birth Certificates for each member of your household other than foster children

􀂄 Marriage License/Certificate (if applicable)

􀂄 Divorce Decree including Complaint for Divorce or Death Certificate for any previous marriages

􀂄 Military Discharge papers (if applicable)

􀂄 Copy of Social Security card

􀂄 Copy of Driver’s License

􀂄 Two pictures of your family and home

􀂄 References (We must have at least three written responses to our requests for references in the file.) If addresses are incomplete or
